Here is how you apply to begin working with an Atlas Team Facilitator. The Team Facilitator will work with you over a six-month period to help guide you through the process of solidifying your dog's skills as a service dog to help you mitigate your disability. The facilitator also helps you to prepare for the Public Access Test leading to certification for you and your dog.

This is what is required for a complete application:

  • A letter of medical necessity from your doctor*

  • A Veterinary Health Check form from your dog's veterinarian*

  • A copy of your dog's rabies certificate

  • A copy of DD Form 214 or active duty ID card if the applicant is a military member or veteran or dependent of a military member**


*We will provide samples of these documents along with instructions for your doctor and veterinarian

**By obtaining a copy of your DD 214, we are able to document the number of veterans in our program. This is helpful for purposes such as applications for funding. You are not required to provide the DD 214, and information on the form, such as character or reason for discharge, is not considered in reviewing your application.
